What’s new for 2025
FitOn Health
$112 per monthly to spend through the FitOn Health platform on gym memberships, in-person classes and fitness equipment, plus unlimited access to on-demand digital workouts.2
$0 Routine Eye Exams
Use our in-network providers and pay $0 for routine eye exams. You'll also continue to receive an allowance that can be used on prescription eyewear, contact lenses and more.3
No More Prescription Drug Coverage Gap
You will now have a lower Part D out-of-pocket maximum of $2,000 for your prescription drugs.3
Benefits you’ll continue to enjoy
Dental coverage
Including exams and cleanings, X-rays, extractions, fillings, and dentures on select HMO and PPO plans.3 Out-of-network dental services now available on select HMO plans.
Vision coverage
Eye exams and screenings, plus an allowance that can be used on prescription eyewear, contact lenses, and more.3
Hearing services
TruHearing® Hearing Aid Program provides guaranteed low pricing on hearing aids and a $0 copay on a routine hearing exam.
On most plans.
Meal benefit
Mom’s Meals® provides 2 meals a day for 14 days after a stay at an inpatient hospital, skilled nursing facility, or other inpatient facility.
Non-emergency medical transportation
Up to 24 one-way rides per year to medical appointments and other health-related services.

Personal emergency response system
A wearable device with one-button emergency calling plus fall detection and GPS tracking on most devices.
In-home assistance
Support services include meal prep, bathing, medication reminders and more.
